In our endeavor to meet the individual requirements each resident is allocated a member of care staff as a ‘key worker’. This is undertaken within the first week of a resident’s stay although if a resident wishes to change their key worker at a later stage this can be easily arranged. The key worker acts as a focal point for the resident and their relatives and will try to ensure that the resident’s personal requirements are not overlooked in the busy day-to-day environment.
To assist staff in this direction the key worker and the resident (with the help of a relative/friend/social worker) will complete a resident Care Plan. The Care Plan specifies in detail the kind of care a resident requires (physical, emotional, spiritual etc) so that all the care staff are then aware of the resident’s own choices, preferences and needs. It is then the responsibility of the staff and key worker to update the Care Plan as and when required. A formal review of a resident’s progress will usually also take place and this will be explained in greater detail prior to admission.
RESIDENTS ARE ALSO REMINDED THAT THE HOLLIES HAS AN ‘ACCESS TO RECORDS POLICY’ WHICH MEANS THAT THE BOTH THE RESIDENT AND INDIVIDUALS NOMINATED BY THE RESIDENT CAN HAVE ACCESS TO THE WRITTEN RECORDS.
The only exclusions to the above is when third parties have explicitly made known to The Hollies that their views and/or correspondence should not be made available to anyone except The Hollies staff. Obviously the home has to respect this request by removing such items from the file prior to ‘open access’.
The Care Plan also incorporates a resident risk assessment, which details exactly what a resident is capable of undertaking and what tasks pose a greater risk.
